The accident occurred on 16th March, 2016 at the University of Hawaii, Manoa. A tank containing highly pressurized and inflammable hydrogen, oxygen and carbondioxide mixture caught fire due to static electric charge. The presseure gauge is considered initiating the static charge and consequently the tank got ignited. Tank, researcher and pressure vessel non was grounded. According to investigation reports, the most probable cause for static charge initiation is considered the statistically charged researcher. As she touched the gauge, charge transferred resulting in bushing discharge which ignited the tank, injuring the researcher, destroying the whole lab, the hall way and the neighboring labs as well.
It was invested that the institution labs did not have sufficient laboratory safety approach.
